Billing and PaymentsIt is important to remember that as a patient you are ultimately responsible for payment for the services provided to you at Mountain View. Charges are payable at the time of treatment. For your convenience, we accept Visa and MasterCard, cash and in-state checks. For your convience you can now pay online: We extend a courtesy discount for uninsured patients who pay in full at the time of service: 20% discount for patients who pay with cash and a 15% discount if paid by credit card. There is no discount for those who pay by check. Lab fees are not subject to this discount. Additionally, as a courtesy to our patients with health insurance, we will submit your insurance claim for you, and will make reasonable attempts at rebilling when necessary, to receive your full contracted policy benefits; however, you are personally responsible for the balance on your account, so we encourage you to contact us if you have not received an explanation of benefits (EOB) for payment from your insurance company within 30 days of services rendered.
Copayment Policy NoticeCopays: Payment for your insurance copay is required at the time of service. (We accept cash, checks and credit cards (VISA and MC) You may be asked to reschedule your visit if you repeatedly fail to pay your copay at the time of service. Mountain View will add a late fee to your bill if the copay is not paid by the end of the day of service.
